Procedure
| Sign into your study app |

| Enter your passcode twice |

| Tap Profile |

| Scroll down to Documents and tap Sign documents |

| Tap Sign next to the document that needs signed |

Review the document and tap Next when ready
TipPinch the screen to zoom in or out. |

| Enter your passcode |

| Enter your name (additional roles may be requested in addition to or in place of the participant (i.e. a caregiver or observer), depending on the configuration of the document) and tap Next |

| Provide your initials and/or signature using your finger (additional roles may be requested in addition to or in place of the participant, depending on the configuration of the document) and tap Next |

| Provide the necessary responses (signature, initials, checkbox, and/or text box) |

| Tap Complete once you have provided all responses |

Additional Notes
- The type of responses needed will vary per document
- Once a document has been signed, it will be available for review at any time within the Documents list of the Profile section of the Participant App
- Documents that do not need to be signed will say View
- Documents that have already been signed will say Signed
- Documents that need to be signed will say Sign
